Art Club Application - First semester

Dear Parents and Students:
I am very excited to say we will begin our after-school Art Club on Monday, October 5, 2009. The Art Club will only be open to fourth and fifth grade students. We will meet every Monday from 3:10 to 4:00 p.m. Due to space and having art only one day a week, Art Club will be opened to the first 25 students that sign-up.
There will be a $15.00 fee for each student per semester. The art fee will not be refundable if your child chooses to leave, or is dismissed from the club. The money will be used to buy art supplies and additional items used only by the Art Club.
Important information
This is an extra-curricular activity, so any discipline issues will result in dismissal from the Art Club.
Students that wish to join must maintain a consistent work ethic grade of 3 or 4 in all areas. Art Club
students must maintain their academic grades at 70 or above. Students with failing grades will be put on
restriction for 3 weeks and may need to attend after school tutoring with their classroom teacher.
There are no refunds for late dismissal, behavior, or grade problems. Students must have an excused
absence from Art Club. Two unexcused absences will result in being removed. (An excused absence
includes: missing school the whole day, going home sick, doctor’s appointment, or special tutoring.)
All students must be picked up on time. After two times of being picked up late, your child will be dismissed from the club. Club meets 3:10 to 4:00. Any students that are not picked up by 4:10 will be sent the After School Program. You will be charged the drop-in fee of $15.00 upon your arrival.
